Staying socially connected and active in the community plays a vital role in maintaining emotional wellbeing, cognitive health, and overall quality of life. Our Social Support services assist people to remain engaged with others and involved in meaningful activities outside the home.
Services may include accompanied outings to attend appointments, do shopping, join social groups, or simply enjoy recreational activities. They also cover in-home social support, such as companionship visits or help with communication, for those who may be isolated or home-bound.
All supports are tailored to individual needs and preferences, promoting independence, confidence, and community connection.
Social support services are specifically funded to provide assistance to people to participate in the community and feel socially included, as an individual or at a group level, and is designed to develop, maintain or support independent living and social interaction. Accompanied shopping and other errands are categorised here.
- Accompanied shopping/errands
- Accompanied day-to-day living needs
- In-home companionship social visits
- Client welfare checks
- Accompanied to appointments, friends or family visits, social and community activities
- Accompanied Recreational Support – this support includes actively participating in activities with the client, such as accompanying clients on walks, swimming/hydrotherapy, exercise classes, craft/hobby activities, cinema, community club activities, going for lunch/coffee, gyms, local markets and other recreational activities.
Flair For Care Additions
We can provide flexible tasks, supports or services that include the following additions to a recipient’s Domestic Assistance or Social Support service type. Which support type the extension applies to is up to the recipient and what funding the recipient prefers it to be invoiced against.
- Personal Care Support – this support is used when clients require assistance to change clothing or require other personal care during social support services, such as swimming/hydrotherapy.
- Accompanied Wellbeing Support – this assistance includes accompanying to medical appointments such as hospitals, specialists, doctors, radiology, blood/urine collection centres, vaccinations etc and also involves making follow-up medical appointments, filling scripts, organising further requested appointments/actions with other services, assistance with filling out legal/government forms
- Technology Use – this support provides access to a laptop computer, tablet or phone to enable a client to access online or phone services such as Telehealth services, meetings or to access accounts such as MyGov from the safety and privacy of their own homes.
- Advocacy & Wellbeing Support – this support includes tasks such as attending, supporting and advocating for client’s interests at appointments (medical, legal, psychological or personal) and other scheduled meetings, liaising with care coordinators, allied health practitioners and other businesses, supporting clients with mental health issues when receiving equipment or supports, using interpreters for non-english speaking clients where required to inform client of appointments or other activities, follow-ups and directions for safe and effective care and support.
- Mental Health & AOD support – this support includes in-home peer support such as companionship, playing games/cards, watching movies, chatting, going for a walk or accompanying clients to social and recreational activities for peer support, companionship and assistance.
- Legal Attendance Support – this service provides support to a client if they would prefer to have a support person present to attend a police station, court or mediation meeting.
